650 HEALTH AND PSYCHOLOGICAL SERVICES
650.1 Health Services
650.1.1 Mission Statement
  The mission of Health Services is to provide timely and appropriate primary and acute health care; to provide effective wellness, disease and injury prevention education; and to provide campus consultations that equip the students to achieve and maintain lifelong health and well-being. Health Services is committed to the ongoing assessment and refinement of its services and programs
650.1.2 Health Services Programs
  Health Services provides primary health care to registered Cal Poly students on an outpatient basis. Services that are provided at no charge include:
  • Primary physician and nursing care
  • Routine laboratory tests
  • Required physical exams
  • Specialty clinics in dermatology, optometry, gynecology and orthopedics
  • Routine x-ray procedures
  • Allergy injections
  • Health education programs
  • Oral health consultation
  • Disease and injury prevention

Pharmacy items, immunizations, orthopedic supplies, and some lab tests sent off campus for processing are provided on a low-cost basis. Most services are available by appointment. Urgent care is available on a walk-in basis.

The Health Services’ medical records are confidential. Information will be released only with the patient’s written consent. Exceptions are a legal subpoena and some communicable diseases which, by law, must be reported to the Public Health Department.

Currently-enrolled students who possess a valid student identification card are also eligible for health services at other CSU campuses. Concurrent and special program students may use Health Services on a low-cost, fee-for-service basis. University staff and faculty are not eligible for services except for on-campus first aid, Workers’ Compensation, consultation for campus-related overseas travel, or physical evaluation as requested by Human Resources.

650.2 Psychological Services
650.2.1 Mission Statement
  The mission of Psychological Services is to assist students in their academic achievement towards graduation by encouraging learning and personal development through resolving social and personal problems, as they are related to membership in the academic community.
650.2.2 Psychological Services
  Psychological Services provides an array of programs designed to support students in academic and personal growth. The services are provided within an educational framework, and include crisis intervention, psychological assessment, and individual and group counseling. All services are personalized, confidential, and available at no additional cost.
650.3 Employee Assistance Program
650.3.1 Mission Statement
  The mission of the Employee Assistance Program is to provide innovative, quality programs and services in a caring, confidential and professional manner, recognizing the diverse emotional, social and cultural needs of campus employees and their families.
650.3.2 Employee Assistance Program
  The University recognizes that there are a number of problems that can affect work performance. This service provides confidential individual counseling, consultation and intervention to all employees.
